How to apply for your investment and join the annual cashback service In joining our services you are deemed to have accepted our terms and conditions. Simple work through the 4 simple steps below: 1. Obtain and complete application forms for your required investments. You can usually obtain forms from the product provider directly. Please make any cheques payable to the product provider for your investment - we do not handle client money. If you would like us to send you the application pack/quotation please send us a £10 fee in advance payable to "UK Life Direct Ltd" If you want to know how much discount will be applied to your investment before you invest please call 0800 970 0457 between 9am and 5pm Monday-Friday or simply email your fund choice to ifa@lifedirect.co.uk 2. Complete our 'new investments service application form' This is where you acknowledge this is an 'execution-only' transaction and supply your bank details to enable us to make annual commission payments to you:  Download as pdf file Download as word document3. Supply identification if your new investments total over £50pm. Under UK money laundering guidelines we are required to identify investors where the investment is over £50pm. Please send certified copies of 2 forms of ID for each investor i.e. passport or driving licence plus a utility bill or bank statement showing your current address. If you send originals we will return these via normal post unless you send a £5 fee to cover the cost and handling of us returning the items by Special Delivery. 4. Please post these forms to us together with a fee if applicable. As a reminder you do not need to pay a fee if you have obtained your own application form and the product pays an annual trail commission. Our address is : UK Life Direct Ltd The Barn, Riverside Business Park Natland Road, Kendal Cumbria, LA9 7SX Please note that if any investment you require does not pay trail commission will will require a £25 fee payable to "UK Life Direct Ltd" before processing your application. You can usually check this yourself by reading the 'Key Features' document for the product. Investment bond applications will be processed with the trail commission option included (e.g. for a Standard Life bond you will get 5.13% enhancement to your investment plus a share of 0.5% trail commission rather than us provide a higher enhancement at the expense of the trail commission). What happens next? We will acknowledge receipt of your application(s) in writing, forward any plan documentation to you on receipt, and then contact you each year with details of the annual commission payment made to your bank.
